Change of Parental Responsibilty ?

Hi, hoping for some advice.

I have twin boys to my ex wife but due to their very premature birth 1 of them has very complex health issues. He is none verbal and cant do anything for himself and is 17 yrs old. Due to his needs I have nearly nothing to do with him and only see him when hes in respite hospice care normally once per month.

I recently received a letter regarding his care plan showing him with a different surname (mothers new married name) and when I have questioned this with social care professionals they have advised me to get legal advice.

We have split, divorced etc and I have paid the required monetary amounts for 7 years and my other boy is with me regularly with his surname being untouched.

My parental responsibilities have been questioned but repeated requests to his mother and social services for access have been ignored. Admitedly I dont want to go down an expensive and possible fruitless legal route.

I guess my questions are...

1. Can PR be removed without my knowledge or consent, and does it allow a surname change ?

2. If so am I still financially responsible ?

My understanding is a child can change their surname from age 16. But I imagine that due to your sons complex needs, he wouldn't have had a say in this.
Unfortunately I think you'd need to go down the legal route and find a solicitor that deals with PR to find out about this.
 
Ex wife has entered a civil partnership with her new fella, I have no axe to grind, but wondering, does he now have formal parental responsibility for our 9yo daughter? Seems like a way to circumvent a court order if so
